These Terms of Service ("Terms") govern your use of empowermed.us (the "Site") and the clinical services offered by Empower Med, PLLC ("Empower Med," "we," "us"). By using the Site or engaging our services, you agree to these Terms.
Empower Med is a physician practice providing preventive medicine, metabolic optimization, hormone optimization, and diagnostic lab services. Clinical services are provided by Dr. Scott Everson, DO, MPH, licensed in Virginia. You must be located in Virginia at the time of each clinical encounter to receive care.
Clinical services require a physician-patient relationship established through the consultation and informed-consent process. Use of the Site does not in itself create a physician-patient relationship.
Content published on this Site, including service descriptions, pricing, educational articles, and FAQs, is provided for informational purposes only. Services are offered to adults (18 years of age or older) located in Virginia. Minors are not eligible for services through Empower Med.
Fees for clinical services are presented at the time of booking or enrollment. Pricing is transparent and itemized. Medication billing is governed separately based on program (GLP-1 medications billed by pharmacy direct; hormone medications billed through the practice with professional-service markup; labs billed at Quest client cost + 50%).
Clinical memberships may be cancelled at any time without penalty. No refund is issued for fees already paid. No further medication is authorized after membership ends. Please see the pricing page for the complete cancellation policy.
The Site, including all content, design, typography, logos, and software, is owned by Empower Med or its licensors and protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works without prior written permission, except for personal, non-commercial use.
To the maximum extent permitted by law, Empower Med’s liability for any claim arising from or related to the Site is limited to the amount you paid us, if any, for access to or use of the Site in the twelve (12) months preceding the claim. Clinical service liability is separately governed by the applicable professional-service agreement and state law.
We may update these Terms from time to time. Material changes will be reflected in the "Last updated" date above. Continued use of the Site after an update constitutes acceptance of the revised Terms.
These Terms are governed by the laws of the Commonwealth of Virginia, without regard to conflict-of-laws principles.
