Testosterone Therapy: Clinical Policy

Testosterone therapy is a powerful intervention for men with documented testosterone deficiency or significant clinical decline. It requires ongoing medical oversight, consistent monitoring, and patient commitment to the protocol. This policy outlines what Empower Med requires to deliver safe, effective testosterone replacement therapy.

Benefits of Testosterone Therapy

When appropriately indicated and well-dosed, testosterone therapy addresses:

  • Low energy and fatigue
  • Decreased libido and sexual function
  • Depression and mood disturbances
  • Poor sleep quality and recovery
  • Cognitive fog and mental focus
  • Unfavorable body composition (fat gain, muscle loss)
  • Bone density decline

Risks and Important Considerations

Testosterone therapy carries real clinical risks that require monitoring and patient awareness:

  • Elevated hematocrit (polycythemia): Testosterone increases red blood cell production. Elevated hematocrit increases thrombotic risk and may require periodic blood donation to manage.
  • Cardiovascular considerations: Testosterone affects hemodynamics, lipid profiles, and cardiac function. Baseline and periodic cardiovascular assessment is required.
  • Prostate health: Prostate screening (PSA, digital rectal exam if clinically indicated) is required before therapy and periodically during. There is no evidence that testosterone replacement therapy causes prostate cancer, but monitoring is essential.
  • Sleep apnea: Testosterone can aggravate or unmask sleep apnea in susceptible men.
  • Fertility: Testosterone therapy suppresses endogenous sperm production and may reduce fertility. This effect is typically reversible after discontinuation, but fertility is never guaranteed.
  • Testicular atrophy: Testicular size and firmness may decrease during therapy due to suppression of LH.
  • Skin and hair: Testosterone may accelerate male-pattern hair loss in genetically predisposed men and increase skin oiliness.
  • Injection site considerations: Injectable testosterone carries local risk (pain, abscess) and transmission risk if topical delivery is chosen.

How Testosterone Therapy Is Monitored

Empower Med uses a comprehensive lab-driven monitoring framework. All therapy begins with baseline labs and continues with periodic reassessment:

Initial Lab Panel: Total Testosterone (MS), Free Testosterone (MS), Bioavailable Testosterone (MS), SHBG, CBC, Comprehensive Metabolic Panel (BMP), HbA1c, LH, PSA, TSH, Lipid Panel.

This baseline establishes your pre-therapy physiology and identifies contraindications before therapy begins.

Annual Monitoring: Repeat LH, PSA, Lipid Panel, Total Testosterone (MS), and CBC. Additional labs (hematocrit, cardiovascular markers) are ordered as clinically indicated by protocol response, symptoms, or risk.

Ongoing monitoring ensures your protocol remains safe, your testosterone levels remain in physiologic range, and your body is responding well to therapy.

Clinic Policy: Lab Monitoring

Testosterone therapy cannot be delivered safely without laboratory oversight. Labs are non-negotiable and ordered through Empower Med at intervals defined by your protocol.

  • Initial labs must be completed before therapy authorization.
  • Annual labs are required to renew your prescription.
  • Additional labs may be ordered at any time based on clinical changes, symptoms, or protocol adjustment.
  • Lab results are reviewed by Dr. Everson personally. You will receive interpretation and any protocol adjustments via your patient portal.

Clinic Policy: Clinical Compliance

Testosterone therapy is a partnership between you and Dr. Everson. Active participation is required:

  • You must keep scheduled lab appointments and protocol review appointments.
  • You must communicate changes in your health, medications, or symptoms via your patient portal.
  • You must respond to Dr. Everson's portal messages within a reasonable timeframe (typically 48–72 hours for clinical matters).
  • If you are unable to comply with monitoring or communication expectations, inform Dr. Everson immediately so your therapy can be managed appropriately.

Clinic Policy: Prescription Refills

Testosterone is a DEA Schedule III controlled substance. Prescription refills are processed exclusively through your patient portal and require clinical review.

  • Submit refill requests through your patient portal with at least 7 days' notice.
  • Refill requests are reviewed by Dr. Everson for compliance with monitoring requirements.
  • No refills will be authorized outside of an active monitoring relationship.
  • Emergency refill requests over the phone or email are not processed. Plan ahead to ensure continuity.
Controlled Substance Notice: Testosterone is classified as a Schedule III controlled substance by the United States Drug Enforcement Administration (DEA). It carries specific regulatory requirements, monitoring obligations, and legal restrictions on refilling and transferring prescriptions.
